Setting up a DNA test with DDC is quick and simple. Most cases are scheduled within 24 to 48 hours of receipt of the client’s information and partial payment. You many initiate your case either by phone or by mail.

Option 1: Call us toll-free at 1-800-319-9099 and speak with one of our knowledgeable and caring Native American Services representatives. They can take all of your information and set up your case over the phone immediately.

Option 2: Complete our Patient Information Form, which you can request by calling our service team at 1-800-319-9099. A representative will mail or fax the form to you. Please provide information only for those willing to be tested. Mail your completed Patient Information Form to DDC with a personal check for either the full payment or partial payment (deposit). VISA, MasterCard, and Discover are also accepted.
